The safety and wellbeing of clients is a priority at ELevate Sports Therapy. This policy outlines important information regarding treatment consent and client responsibilities.

1. Health Information

Clients must provide accurate and up-to-date medical information when completing registration forms and before treatment sessions.

This includes informing the therapist of:

  • Injuries or medical conditions

  • Medications

  • Recent surgeries

  • Pregnancy

  • Any changes in health status

Providing accurate information helps ensure treatment is safe and appropriate.

2. Informed Consent

Before receiving treatment, clients must complete and sign a consent form acknowledging that they understand:

  • The nature of sports therapy treatment

  • The potential benefits and risks

  • Their right to stop treatment at any time

Consent can be withdrawn at any point during treatment.

3. Treatment Decisions

The therapist may decide not to provide treatment if it is considered unsuitable, unsafe, or outside the scope of practice.

In such cases, clients may be advised to seek guidance from another healthcare professional.

4. Minors

Clients under the age of 18 must have written consent from a parent or legal guardian before treatment can take place.

A parent or guardian may also be required to attend appointments.
